I've just read a bit about LinkFreeze and ISAPI_Rewrite and they both seem like great products but I don't think they'll do what I want. Please advise.

I want to have URLs look like this: (desired)

Instead of ones that look like this: (current)

The mapping of "id=45345" to "/10/05/title-of-page.asp" would be stored in the DB and "/10/05/title-of-page.asp" would not actually exist on the web server. Is this possible with your products?

No, exactly that is not possible. But if you could change default.asp to accept /10/05/title-of-page.asp instead of id=45345 then you could use ISAPI_Rewrite to rewrite request to /xx/yy/somethins.asp to /default.asp.
