When using ISAPI Rewrite, we're noticing a strange
behavior. Most of the time our rewrite rules work exactly
as expected. However occasionally the same URL that works
most of the time will redirect incorrectly.

For example, let's say the URL is
"http://foo.com/mypage.aspx?a=1&b=2&c=3". Normally this
will redirect to "http://wwww.foo.com/mypage.aspx?
a=1&b=2&c=3". Sometimes though
(starting with the exact same URL) the redirect will go
to "http://foo.com/mypage.aspx?
a=1&b=2&c=3,http://wwww.foo.com/mypage.aspx?a=1&b=2&c=3".

When we turn on rewrite logging I notice that for the
requests that redirect inappropriately there's a slight
difference from the ones that redirect properly. There's
a 7
digit number that is normally the 3rd column in the log
but when there's an error, it is "-4".

My assumption is that -4 is an indicator of an error, but
I'm not sure what error it indicates or what to do about
it. The URL passes through several rewriterules but the
one that actually causes the strange behavior seems to be
this (only when the domain doesn't start with www.):

RewriteRule .? http://www.%1%2 [R=301,L]


There's nothing abnormal reported in the error.log file.

Any input on what could be going on or what I can do to
fix it would be great.

edit:
Forgot to mention that this doesn't appear to be OS dependent. It occurs on Windows Server 2008 as well as
Windows 7 (both using IIS 7, I believe)

Such weird issues occur when ISAPI_Rewrite wasn't installed correctly.
Here's an articleregarding "Double registration", which may be causing such issues.

If you check the article and will not have any results, please, provide the full extract from the log file.

Is there an IIS7 version of that article? The dialogs are
somewhat different and the xml files seem to have a
different schema.

Also, to reiterate, the problem is not all or nothing. it
works correctly roughly 80% of the time.

Unfortunately there's no IIS7 article, but we'll consider it in future.
